When every communication channel exists, yet the information still goes nowhere
In a nuclear environment, the quality of communication depends not only on procedures and technology. It also depends on organisational culture: on how the system responds to a question, a doubt, bad news or a warning from outside the usual inner circle.
In my article A Reactor Alone Does Not Make a Nuclear Society, I wrote about the difference between possessing the technology and having a wider nuclear culture. A reactor is its most visible element. Behind it must stand knowledge, responsibility, institutions, professional continuity and the ability to cooperate. Yet individual components do not by themselves create a functioning system. Information must be able to flow between them.
A system may have experts, procedures, organisational structures and every modern communication channel. If information does not move between its parts, the system may look connected from the outside while beginning to disintegrate internally into separate islands.
Organisations do not usually fail because no one inside them knows anything. They often fail because knowledge does not reach the person who needs it, a warning does not reach the level at which it should be addressed, or a question receives no answer because it has been treated as a disruption.
Then the problem is not a lack of information. The problem is interrupted flow.
How the term communication infarction emerged
I began using the term communication infarction long before I entered the nuclear field. It emerged in project work, in moments when progress came to a halt because of a single missing piece of information. The client did not answer the phone. The call was not returned. The email went unanswered. A decision without which the project could not proceed remained suspended somewhere between people for days.
The project existed. The contract existed. The phones, email and other communication channels worked. Yet information did not flow.
For the contractor, such a situation can be extremely burdensome. The deadline continues to run, the work stands still, and responsibility for progress does not disappear. The other party’s missing response becomes an operational problem for the person expected to deliver the project.
At the time, I called this phenomenon a communication infarction. Today, I see that it is not limited to business relationships. It has become a broader organisational and social pattern.
Messages circulate. Understanding does not.
We have never had so many ways to communicate. We are reachable by telephone, email, messaging applications, video calls and social media. Information can be sent to the other side of the world in seconds. Yet we often no longer know how to establish a simple conversation. We do not reply. We do not return a call. We do not say that we do not yet have an answer. We do not ask when we do not understand. We read only far enough to begin composing our response.
Instead of receiving the content of a message, we react to a single word, a first impression or a story we have constructed ourselves. This creates conversations in which everyone speaks, but no one truly engages with another person’s thought.
These are no longer conversations. They are parallel monologues. The communication channel is open, but the content does not get through.
Sent does not mean received
One of the most dangerous assumptions about communication is the belief that our task is complete once we have said or sent something. Yet a message that has been sent is not necessarily information that has been received. The recipient may not have seen it. They may have read it too quickly. They may have understood the instruction differently from the sender. They may not have known that a reply or action was expected.
In such a case, communication exists formally. There is a written record, an email or a completed meeting. Functionally, however, communication did not occur.
Successful transfer of information consists of several stages:
sent → received → understood → confirmed → acted upon
If the chain breaks at any point, communication is incomplete.
Information becomes useful only when it is correctly understood on the receiving end and incorporated into the work that follows. This is why demanding environments require checks for understanding.
What was heard? How was it understood? Who will act? By when? Are the responsibilities clear? Without this, the system operates on assumptions. Assumptions are not a reliable communication protocol.
Silence is information, but it is often ambiguous
In projects, many problems are caused not by an explicitly wrong answer, but by no answer at all. Silence may result from overload, indecision, internal coordination or discomfort with delivering bad news. Sometimes a person does not yet have an answer and therefore says nothing. But that too can be communicated.
We do not yet have a decision. We will send you our answer by Friday.
That sentence is far more useful to a project than several days of silence. It does not resolve the substantive issue, but it enables planning. The other party knows the current status and can adjust the work accordingly.
A short, clear response often prevents more damage than a complete explanation delivered too late.
Communication responsibility does not mean always having a final answer ready. It means saying what we know at that moment, what we do not yet know, and when the other party can expect the next update.
In a safety-critical environment, however, silence has another dimension.
It may indicate that people do not know whether they are allowed to speak up. Whether a question will be welcomed. Whether a warning will be treated objectively. Whether disagreement will bring social or professional consequences. A system in which people learn that silence is safer than asking questions may appear stable from the outside. Internally, it is losing one of its most important layers of defence: a timely human warning.
When a question is treated as a disruption
Communication infarction does not occur only when no one responds. It also occurs when a response arrives but has little or no connection to the question.
This is especially visible in public and organisational discussions. Someone asks a simple question, and the replies begin to address their supposed motives, character, affiliation or future behaviour. A single sentence becomes an entire scenario. We no longer respond to what was written. We respond to our own idea of a person we often do not know at all.
This reveals the difference between reading and understanding.
We can read a text without truly taking in its content. We can select a single word that triggers a reaction and then speak past everything else. The result is a great deal of noise, little information and almost no shared understanding.
In the nuclear field, this pattern is particularly problematic.
A question can quickly be interpreted as opposition. A warning as an attack. Doubt as evidence of ignorance. Professional disagreement as personal conflict. When this happens, communication no longer builds understanding; it hardens existing positions.
Safety culture is therefore not measured only by the existence of rules, procedures and communication channels. It is also measured by how an organisation handles information that is uncomfortable, incomplete or comes from outside the established hierarchy.
If a question is treated as a disruption, a warning as a personal attack, or a different perspective as a threat to group cohesion, the information does not disappear. It merely disappears from the formal flow. The risk remains in the system.
Safety culture reveals itself when the information is uncomfortable
Almost every organisation can state that it supports open communication. The real test comes when someone brings bad news, points out a deviation or expresses doubt about the prevailing interpretation.
Is the information considered on its merits?
Is the first question who said it?
Is the person thanked for raising the problem?
Or does the organisation begin looking for a way to minimise, redirect or silence the warning?
Safety culture is not a culture in which disagreement never occurs. It is a culture in which disagreement does not interrupt the flow of important information. It needs people who are willing to ask questions. It also needs an environment able to receive those questions without an automatic defensive response. It is not enough for an organisation to tell employees to speak up. It must also ensure that they will be heard.
Leadership defines what may be said
Leadership shapes communication culture primarily through its responses. Not only through policies, presentations and declarations of an open-door policy, but through what happens when someone actually walks through that door. If the bearer of uncomfortable information is ignored, socially excluded or treated as the problem, the organisation quickly learns which information is safe to pass on and which is better withheld.
Such learning can be entirely informal. No one issues an instruction forbidding discussion of a particular topic. People simply observe how those who spoke about it before them were treated. Over time, communication becomes selective.
Information that confirms expectations travels upwards. Uncomfortable deviations remain at lower levels, are softened in language or disappear in internal coordination.
Such a system can create the appearance of stability. But stability based on filtering information is not resilience. It is a blind spot.
When connectedness becomes a filter
Strong ties within a professional community can be a major advantage. They enable trust, rapid cooperation, mentoring and the transfer of experience. People who have known and worked with one another for a long time can coordinate effectively even under demanding conditions. But the same connectedness can have another side. When belonging to the inner circle matters more than the content of the information, connectedness becomes a filter.
Voices outside the established network may be overlooked not because of the quality of the question, but because of who asked it. An organisation may then, unintentionally, begin to equate trust with personal familiarity, expertise with belonging, and credibility with personal connections.
In a safety culture, the content of information must carry more weight than the position, background or affiliation of its author. An important warning does not become less important because it was raised by someone outside the usual circle.
Sometimes it is precisely the outside perspective that notices an assumption the internal group can no longer see.
A strong community is not one that accepts only its own members. A strong community is one that can also accept information that did not originate within it.
Communication requires both sides to participate
Responsibility for clear communication does not rest solely with the sender. The sender must formulate the message precisely enough, provide the necessary context and state clearly what is expected. The recipient must read or listen to the message, ask when something is unclear, and respond within a reasonable time.
Both sides must be willing to verify that they are talking about the same thing. Without this, communication quickly turns into the transfer of blame:
I sent it. I said it. They should have known. I thought it was clear.
These sentences often appear only after something has already gone wrong. But the purpose of good communication is not to prove afterwards who sent the message. It is to ensure that the information was understood and acted upon in time.
A clearly worded message does not help if the other party is unwilling to receive it. Likewise, an attentive recipient cannot reliably infer the meaning of a message that omits key information.
Communication is shared work.
Communication as part of the safety architecture
Communication is often treated as a soft skill. As something desirable, but separate from serious technical or project work. In reality, communication is the infrastructure of cooperation and part of the safety architecture.
Requirements, decisions, warnings, knowledge and responsibilities move through it. When that flow stops, work begins to stop as well. A system may have exceptional experts, advanced technology, detailed procedures and a sound organisational structure. If people do not pass on information, receive it or verify understanding, the individual parts of the system remain disconnected.
Each person can perform their own task without sharing the same picture of the state of the whole. From the outside, the organisation may look connected. Internally, it operates as a group of islands. Communication is one of the links between technical, organisational and human layers of defence. It does not replace procedures, training or engineered safeguards, but it enables these elements to function as a system.
When communication breaks down, each individual layer may still exist. Their combined effectiveness, however, is weakened.
In a technical environment, communication is not a social extra
In everyday life, poor communication causes misunderstanding, resentment or lost time. In a complex technical system, the consequences can be far greater. A shift handover, a configuration change, an unusual equipment condition, a work restriction or an unresolved technical issue must be communicated clearly, promptly and without ambiguity.
It is equally important that the information is confirmed. High-responsibility environments therefore use the principles of closed-loop communication, also known as three-way communication.
The sender gives the message. The receiver repeats it back or confirms it in their own words. The sender then verifies whether it was understood correctly and, if necessary, corrects any discrepancy.
This may seem overly formal to some. Its purpose, however, is to reduce the room for assumptions. When responsibilities are significant, it is not enough for someone to think that the other person probably understood. They need to know.
Reliable communication supports defence in depth, maintains shared situational awareness and helps prevent individuals or organisational units from operating on different versions of the same event.
Poor communication is therefore not always merely a personal shortcoming. It can be a systemic vulnerability.
A country with nuclear power must also know how to listen
A country with nuclear power needs more than technical infrastructure and a handful of institutions involved in nuclear energy. It needs a flow of knowledge between generations, disciplines and institutions. It needs clear communication among those who plan, operate, oversee, educate and make decisions.
It must also be able not only to provide information to the public, but to verify whether that information has been understood. Published content does not necessarily produce understanding.
A website, brochure, presentation or public event is a communication channel. Its existence alone does not mean that communication has been established.
If questions go unanswered, professional differences are concealed behind unified messaging, and institutions speak mainly when they need public support, the system is not building a lasting communication culture. It is providing information only intermittently.
Nuclear culture is not built only at the moment when a new project must be explained or a major decision defended. It is built continuously. Through responsiveness. Through acknowledging open questions. Through a willingness to listen to people who do not use the same professional language. And through the ability to accept a question even when it does not come from the expected circle.
A country that wants to sustain a nuclear programme must know how to speak. But it must also know how to listen.
When the channel is not enough
A telephone alone does not create a conversation. An email alone does not create understanding. A meeting alone does not create agreement. Nor does a large volume of publicly available information automatically create an informed society. Communication exists only when information moves between people, is correctly understood, and leads to a clear next step.
A country with nuclear power is therefore not simply a country with a reactor, experts and institutions. It must also be a country in which information can move between people, disciplines, organisations, decision-makers and the public. A country in which a question receives an answer, uncertainty is not concealed by silence, and understanding is verified rather than assumed.
Safety culture is not a culture in which everyone says the same thing. It is a culture in which important information can pass through the system even when it is uncomfortable, unexpected or voiced by someone outside the usual circle.
When the flow of information stops, the system may still look whole from the outside. Its individual parts, however, begin to operate on their own.
This is a communication infarction.
It is not necessarily caused by a lack of words. Often the opposite is true. There are plenty of words. What is missing is flow. And when we speak about a country with nuclear power, projects, safety culture or complex technical systems, the flow of information is not a matter of politeness.
It is a prerequisite for operation.
