SCRAM is often imagined as a dramatic emergency action. In reality, it is a designed safety function: a rapid shutdown of the fission chain reaction by inserting negative reactivity into the reactor core. But shutting down fission is not the same as removing heat. After SCRAM, the system continues to require cooling, monitoring, procedures and control.
Nuclear safety culture is not a slogan, a poster or a formal statement on a wall. It is the way an organization thinks, communicates, questions, learns and responds when safety matters.
Operator error is often used as a simple explanation for failure. In complex technical systems, it should not close the analysis. It should open a deeper question: what made the action possible, likely, or difficult to recover from?
